As last year, a survey will be sent via surveymonkey after summer reading has finished.  The survey will be due September 16.

  • Name of System

  • What was the total circulation of materials from June 1-August 31, 2016?

  • Total Budget with donations listed separately

2016 SRP Surveys / Evaluations

These short surveys are designed to collect information about community participants' views your summer programs. Please print these surveys and provide them to your program audiences as often as possible.

Three new Project Outcome Surveys have been created regarding the summer reading program and are intended to be used at the end of the program or as participants finish the program. The three new surveys are attached below.

If you are already using the SRP surveys above, you may keep using those. If you have not yet started as of July 09, 2016, then you may consider using the ones below for the end of program survey.

PLA also offers guidelines for using these surveys. The FAQ document is also attached below. The FAQ document references the Education/Lifelong Learning and the Early Childhood Literacy Surveys, which are also included above for specific entertainment and educational one time programs you offer.
